I use audio over HDMI directly to my TV. I have a full Ubuntu 10.10 desktop install. I can hear the audio from both my desktop and from XBMC but the volume level is VERY low. I have everything in alsamixer set to 100%, I have XBMC set to 0 db and I can barely hear anything unless I set my TV volume to an extremely high level.

Open the audio mixer (it's called pavucontrol here) Switch to the Configuration tab (rightmost tab), There is a dropdown for selecting the profile for the builtin audio device.
