You need to install both the Realtek Vista HD driver (latest version 2.15) AND then the Realtek ATI HDMI Audio Device driver (latest version 2.13) in that order. The HD install is not complete without BOTH and you won't get the screen I showed above, or the Realtek icon without the first. This may be the root of your problem.
