Try every audio option ONE AT A TIME and note performance ( do not ignore the 'one at a time' rule). Repeat this with the video settings (the audio will stutter if the CPU gets bogged down. While it's unlikely that your GPU is the bottle neck, being that it's an emulator, absolutely everything has to go through the CPU).

Audio Lag in PCSX2 : emulation - reddit

The audio lag is significant and there is no way to fix audio lag in Guitar Hero 1-Encore, just visual/input lag. Are there any specific audio plugins or settings I should be using? The audio plugin I'm using right now is the one that came pre-installed with PCSX2. Try changing the SPU2-X module in options, I'm currently using SDL audio.

The XSound Go produces crisp clean sound; even at high volumes I noticed zero crackle or distortion, but for my personal taste in music, I just can't help wishing it could produce a punchier bass. Credit where credit is due: this thing can fill a room with sound, but it doesn't have any richness to elevate its ability to sound exquisite, but ...

Gameplay video is stuttering while maintaining constant 60fps all the time, no drops at all, no sound distortion. ONLY fix so far is enabling vsync thru pcsx2 settings. Standard settings give this problem, disabling/enabling speedhacks don't help. Seems to be some issue with aero/ double buffering/ vsync.

The stand-alone had the same audio lag as PCSX2. The libretro core, on the other hand, seemed to have the audio synced, but the framerate was worse (around 50fps), which resulted in both audio and video quality problems. Thing is, though, I don't experience audio issues with any other game or application.
