This is the fix: Control Panel > Sound > Recording > double click on your microphone > Levels. Lower the Microphone level. I lowered mine to 55. I recorded a conversation between my husband who was accross the room and me sitting right next to the Surface Pro and the recording was impeccable. I hope this works for you.

The only way to make it work is by right clicking the attachment and selecting "Open" this will open the mp4 file outside of the OneNote app, and it will actually play. If we record the audio from the computer, the audio file will work fine …
