MythTV has the ability of suspending Pulseaudio so it can reach ALSA, so getting ALSA to work should just be a matter of selecting the correct output device in MythTV. You're correct, I removed the Pulse Audio Volume Control and by selecting the right output in MytvTV, I got the sound working.
