I had the same problem with a video I downloaded from Vimeo. What fixed it for me was the following: Control Panel>Appearance and Personalization>File Explorer Options>View>Uncheck "hide extensions for known file types". Then go to the file and change the file extension to .mov (or an alternate video extension)

It is a known issue in 12.1.1 version of Premiere Pro and 10.11.X version of the MAC OS where MP4 or MOV files containing H.264 codec import as Audio only.
