iTunes 12.5.2 My EQ was off and I was still getting this distortion. Somehow the "Sound Enhancer" was turned up to high. Once I fixed that, no more distortion. Go to iTune Preferences > Playback. You will see the "Sound Enhancer" slider and an option to turn it off. Just turn it off, problem fixed
