I’m having an issue since applying the April 2019 update to my Vero4K. The audio drifts horribly after watching about 15 to 30 seconds of tv. The HDHomeRun has been updated to the latest firmware. I’ve tried watching the HDHomeRun directly from my TV, and the audio sync is fine. Also, before the April update, it worked fine through the Vero4K.

Streaming MPEG2 video from my DLNA server is often choppy with that enabled. It's never affected the audio, but the DLNA service might be smoothing that out.

On other platforms, the HDHomeRun app actually has its own video player, that can be tweaked and made to work with all the different broadcasters. When there's an issue with something like Android or iOS, the app can be updated and those issues can be fixed. In other words, use something else if you really want good short term ATSC 3.0 support.
