Most sound problems seemed to happen after the patch. Why don't you guys roll back to the original version? It can be done quite quickly (10 minutes, worst case scenario) by deleting a few files in GTA IV directory (GTAIV.exe, GTAIV.exe.cat, GTAIV.exe.cdf, GTAIV.exe.cfg) and repairing the game via the ad hoc windows menu.

System Sound/Playback from Start>Control Panel>Sound>Playback tab and make sure everything is working correctly that way in their resepected properties tabs. I really don't think it's the system sound as much as the game. There could be some sort of conflict between the game and audio driver that isn't showing up.
