I was using a august 2007 version of FFDshow and upgraded to latest version and still the same problem. If I do not upscale and just let it use Noise reduction and post processing then the videos play fine.

I'm using MPC-HC with the mid range settings set up in FFDShow. If I go to my default profile, lipsync is fine. When I change to my DVD Upscaling profile, it seems like there is a very slight lipsync issue. I'm using SPDIF audio (older reciever, no HDMI) and using the FFDShow audio decoder for digital audio output (passthrough of AC3 or DTS).
