Control Panel > Hardware Sound > Manage Audio Devices > Speakers > Sound Blaster (tab) > Settings (button) > then selected the correct speaker configuration. Restart Fallout. I also could not hear the sound of footsteps or sheathing/unsheathing a weapon. Fallout New Vegas seems to get kind of wacky if you're using 5 channel audio (5.1 surround sound or higher). If you've set anything like that up recently, go back into your audio manager and try messing around with settings (revert to stereo to test) and see if that fixes it.
