After installing that, you'll need to right click the sound icon> go to recording devices > right click show disabled > and enable stereo mix. If its there already just enable it. How this works is realtek will decode and playback will be through the hdmi device (nvidia hd audio) so two heads better than one.

It apears to be a vista issue. Simply run the .exe file in xp compatibility mode. I also changed the sound option to stero sound, but I don't think that mattered. I'm using onboard sound, so I know its not just a matter of needing some supped up sound card. In case you don't know how to run it in compatibility mode: right click on the executable
