Try the following. Open Avid and launch a project (any project). Launch the audio tool and try and output tone. If this works, try playing a clip or sequence. You should have audio working for the rest of your session. (I know this sounds like black magic, but I stumbled across this "fix" when I was trying to sort out issues with a Realtek chipset.

The track is there just how you left it, no changes, yet no playback. Kind of dumb but here's the solution that works: create another audio track and copy everything from the first track to it, waveforms, plug-ins, and assignments. This doesn't explain why it happens, but it does keep you working.

The tricky thing is that Inst tracks don't show you the MIDI routing details by default. You have to turn that on in the View menu: View>Mix window>Instruments should be checked. You can turn it on in the Edit window, too, but I find that to be a waste of space..
