For those that still can't get AudioFX to work on CM12.1, I think I found a workaround (on Galaxy Note 3 (HLTE) nightly build). Start the CM music player and some other player like Play Music or install an additional one (e.g. Shuttle+, Jair Music Player Beta) and start it. Make sure your AudioFX mod is enabled

Weird as I am also on CM12.1 Nightlies and AudioFX works just fine. What build is yours, date? I only had that problem on COS12/.1 where the new version of AudioFX didn't work, but I have never had that problem on any of the CM12.1 nightilies this far.
