After some googling, I found a fix you can try, worked for me at least. I had the same problem, flv1 worked but flv4 had no video. Apparently, the latest official version of the Gabest FLV Splitter, 1.001 has not been updated to recognize flv4. I found a more recent, unofficial build by celtic_druid that worked (note its version# is still 1.001).

That's odd, Case. My MediaInfo Mac (7.7.58) says that "flv" has AVC video and AAC LC audio. Yes, VLC will play it. I haven't found any "export" settings that work. Unlike every other flv of my acquaintance, neither iSquint nor its big brother VisualHub will touch it. MPEG Streamclip is like QT. No video. FfmpegX doesn't recognize it. Curious.
