You may wish to consider using the 3CX SBC. One way audio issues seem to be quite common when a number of remote extensions are run behind the same router with no SBC used. It can also depend on your router. In the mean time (as suggested) try checking the option in 3CX (per extension), PBX delivers audio.

Each extension with a remote STUN phone has the option “PBX Delivers Audio” ticked, On the remote site, each phone should have a static local IP address. I have t-28 and VP2009. 3cx jusy re-published how to set up remote extensions on the blog. You may want to revisit this and I suspect how the routers and/or firewalls are set up at both ends. I assume that when a remote extension initiates a call, the other side gets the ring, answers and then silence.
